The Special Needs Plan Alliance has an array of formal partnerships as well as working relationships with outside organizations. Below, the Alliance has compiled resources from these organizations or linked to their websites.
- Home | Integrated Care Resource Center: Funded by the Medicare-Medicaid Coordination Office (MMCO), these resources are targeted to states interested in developing integrated programs. The SNP Alliance often joins meetings with member plans and states to discuss ICRC resources and collaborates with ICRC on targeting resources to states. Also, specific ICRC materials are linked to with our D-SNP operations webinar during which the speaker discusses what plans and state elements are needed for Coordination-Only D-SNPs (CO D-SNPs), Highly Integrated Dual Eligible D-SNPs (HIDE), and Fully Integrated Dual Eligible (FIDE) D-SNPs.
- The Link Center | ACL Administration for Community Living which provides in depth information and best practice for delivering care to persons with co-occurring disabilities, I/DD and Severe Mental Illness
- Center for Health Care Strategies | Better care where it’s needed most (chcs.org) Better Care Playbook: Funded by seven foundations, this website offers a wide array of evidence-based services and supports which could be used as Special Supplemental Benefits for the Chronically Ill (SSBCI). Initially, members may search the CHCS site and request SNP Alliance assistance with identifying SSCBI. Later, a tailored search tool will be launched.
- ASAM – American Society of Addiction Medicine: This the professional organization for physicians and other clinicians specializing in addition medicine, Under an agreement with ASAM, certain licensed ASAM materials are being made free to only SNP Alliance members.
