Now that Special Needs Plans have achieved permanency, our important work of advancing integration continues forward. This year’s SNP Alliance Fall Forum, Real World Solutions to Complex Care: Integration Post Permanency, will, once again, bring together Special Needs Plans (SNPs), Medicare-Medicaid Plans (MMPs), state and federal policy leaders and other key stakeholders for a dynamic and interactive day-and-a-half meeting. This Forum will, indeed, focus on “real world solutions for complex care” by bridging care management, behavioral health, integrating social determinants data into care processes, and providing critical updates for policy and regulatory guidance. This vital Forum ties together plan operations, clinical care models, state and federal policy in a unique environment of shared learning. We encourage you to bring your executive, operations, policy, quality, care management, behavioral health, and clinical leadership teams – there will be something valuable for everyone.

Meeting Highlights include:

  • Updates from Congressional staff on furthering integration and other related federal policy issues for specialized managed care plans.
  • In-depth discussions with state and other key leaders on state’s view of furthering integration, including both challenges and potential solutions.
  • Updates on quality measurement and progress toward recognizing social determinants of health.
  • Break-out session for shared learning that will address C-SNP and I-SNP integration, complex care management for I-SNPs, partnering with community organizations for behavioral health, and aligning measures to process improvement.
  • Addressing marketing and “pre-enrollment engagement” strategies to address the needs of high-risk and high-cost consumers and support the value to them of integration.
  • An overview of supplemental benefits and options/challenges presented.
  • Open discussion with CMS leadership and staff regarding key regulations and guidance.
